Request for Pick-Up of Assessments

The packets should be collected at a central location in your building and shipped together in a box (not provided - a large paper box would work perfectly) at the end of each assessment. When all participating teachers have returned assessments to the box located in your building, please use the following link to arrange for a pick-up date from UPS. Please check the following items when you return the tests:

1) Make sure that each test has the correct student ID number. It is critical that the tests DO NOT contain any student names, only ID numbers.

2) Please check over them and make sure they are complete. Double-check to make sure no student names are listed on any test.

3) The pre-test should be returned to us in the envelope provided. At the end of the module, the post-test should be returned in the provided envelope.
